Step 1: Register on GotSport Platform
NOTE: ASA [Arizona Soccer Association] is moving to a new platform this season. Your emails for your login from last season on GotSoccer.com will still work on the new GotSport platform. Here’s the GotSport Registration Link:
Step 2: Birth Certificate & Medical Release Form
Additionally, we will need the following sent to state48fcforms@gmail.com: 1. Copy of your player’s birth certificate 2. Signed medical release (Medical releases must be notarized as well) Digitally sign the form on GotSport.com. Print THIS form to have it notarized).
Step 3: Complete State 48 FC Forms
Please download and review all forms, then complete the Player Information form & sign and return the signature page to state48fcforms@gmail.com
Step 4: Send an Electronic Headshot
Send an electronic headshot for your child’s player pass. (Can be the same photo from TeamSnap) to state48fcforms@gmail.com.
Step 5: Order Kit
Please confirm with your team manager that your player’s requested number is available and assigned to your player prior to ordering. NOTE: Backpacks are NOT mandatory but are available for purchase through the team store. There is an option in the team store to purchase additional practice gear if you would like to purchase more than what comes in the uniform kit.
